Don Mills: community workshops regarding health promotion, education materials

Recommendations:

Utilize the EMR to better track screening status and create proactive reminders. Utilize SAR data to improve data quality. Similar to breast cancer screening initiatives. Tracking and auditing EMR data proves to be effective in identifying eligible patients. Letters from CCO seem to get better responses.

  • Cervical cancer screening clinics have been conducted and continue to be conducted by our NP to allow for easy access and appointment time slots for patients wishing to bring their cervical screening up to date
  • Patients continue to be notified of overdue screening at each visit
    -> post signage within our clinic and on website regarding testing and how to tell if you are overdue
  • Participated in the Cancer Care Ontario screening calls pilot project -> during which we conducted phone calls to our overdue/never screening patient population so that we may inform them of screening guidelines
    • Continue to improve EMR so that when we run our reports we are getting better quality data ? also have applied for funding to help support our QI improvements within our EMR
    • Working towards better communication with Provincial Preventative Care initiatives as lists are inconsistent
